This is really easy to look up, btw. Just go to any firm's site and look through any docs/research reports/etc. They will always have contact info listed, and the general format's the same for everyone at the firm.
Is it good to send out thank you emails? I know for banking interviews it is pretty much useless in terms of showing interest, so I don't even bother.

What happens if people have a really common name, like John Smith. Should you ask them for their email address (for thank you notes, etc)?
If it's someone important enough to send a thank you note to, you should always get their card or ask for their info. Protip: ask if they'd be open to discussing [that cool project/similar experience as you/whatever] in more detail later. That gives you an easy reason to followup.
The only "interview" I've had so far where I didn't have their contact info before was a phone interview for Insight and timing made it hard to ask for contact info (20 mins total). I wrote down the interviewer's name, though, and it turned out to be okay despite the person's commonish name.
fyi, for future reference, WSO Company Database has e-mail convention/structure for thousands of companies all publicly available :-)
